How to Do It
From the My Practice page, click the Providers tab.
Now, use the Search function to find the provider in question, then choose Deactivate.
This modal will appear, explaining deactivation and future accessibility in the Inactive tab. Click Confirm.
You can now view the deactivated provider in the Inactive tab. From here, you can either reactivate or delete the provider.
